Certain medical conditions may complicate a pregnancy. However, with proper care, most women can enjoy for a healthy pregnancy, despite their medical problems.
Diabetes before pregnancy:
Diabetes is a condition where the amount enough insulin or is not produced or the body is unable to use insulin produced. Insulin is the hormone that allows glucose to enter body cells provide energy. When glucose can not enter cells, accumulates in the blood and the body's cells literally starve. Diabetes during pregnancy can have serious consequences for both mother and fetus. The severity of problems often depends on the degree of diabetes in the mother, especially if you have vascular (blood vessels) complications and poor control of blood glucose.
Diabetes that occurs during pregnancy is described as:
Gestational diabetes - when a mother who does not have diabetes develops a resistance to insulin because of pregnancy hormones. Women with gestational diabetes may be non-insulin dependent or insulin dependent.
Pre-existing diabetes - women who already have diabetes and pregnancy.
What is gestational diabetes?
Gestational diabetes is a condition in which the glucose level is elevated symptoms of diabetes and others appear during the pregnancy in a woman who has not yet been diagnosed with diabetes. In most cases, all diabetic symptoms disappear following delivery. However, women with gestational diabetes have an increased risk of developing diabetes in the future, especially if they are overweight before pregnancy.
Unlike other diabetes, gestational diabetes is not caused by lack of insulin, but by blocking effects of other hormones in the insulin that is produced, a condition known as resistance to insulin.
What causes gestational diabetes?
Although the cause of gestational diabetes are unknown, some theories about why the condition occurs.
The placenta supplies a growing fetus with nutrients and water, so it produces a variety of hormones to maintain pregnancy. Some of these hormones (estrogen, cortisol and human placental lactogen) can have a blocking effect on insulin, which usually begins around 20 to 24 weeks of pregnancy.
As the placenta grows, more of these hormones are produced, and insulin resistance comes to be higher. Normally, the pancreas is able to produce extra insulin to overcome insulin resistance, but when the production of insulin is not enough to overcome the effect of placental hormones, gestational diabetes results.
What are the risk factors associated with gestational diabetes?
Although any woman can develop gestational diabetes during pregnancy, some of the factors that may increase the risk include:
history diabetes family
obesity
have been previously delivered a large baby, stillbirth, or a child with a birth defect
age (women over 25 are at greater risk than younger women)
Although increased glucose in urine is often included in the list risk factors are not believed to be a reliable indicator for gestational diabetes.
How is gestational diabetes diagnosed?
A glucose screening test is usually performed between 24 and 28 weeks of pregnancy, which involves drinking a glucose drink followed by measurement of sugar level blood after an hour.
If this test shows a level of increased blood sugar, three-hour test glucose tolerance test can be performed after few days following a special diet. If the results of the second test are in the abnormal range, gestational diabetes is diagnosed.
The Treatment for gestational diabetes:
Specific treatment for gestational diabetes will be determined by your physician based on:
Your age, overall health and medical history
extent of disease
Your tolerance for specific medications or therapies
expectations for the course of the disease
your opinion or preference
Treatment for gestational diabetes focuses in maintaining blood glucose levels within the normal range. Treatment may include:
Special diet
exercise
blood daily glucose monitoring
insulin injections
Possible gestational diabetes complications for the baby:
Unlike other types diabetes, gestational diabetes generally does not cause birth defects. Birth defects usually originate sometime during the first trimester of pregnancy. It is more likely in women with pre-existing diabetes, there may be changes in blood glucose during that time. Women with gestational diabetes generally have normal blood sugar levels during the critical first trimester.
The complications of gestational diabetes are usually manageable and preventable. The key to prevention is careful control of blood sugar levels as soon as the diagnosis of gestational diabetes.
Babies of mothers with gestational diabetes are vulnerable to several chemical imbalances, such as low serum calcium and low serum magnesium levels, but in general, the main problems gestational diabetes are:
macrosomia Macrosomia refers to a baby that is considerably larger than normal. All the nutrients the fetus receives come directly from the mother's blood. If the maternal blood has too much glucose, the pancreas of the fetus senses the levels high glucose and produce more insulin in an attempt to use this glucose. The fetus converts the extra glucose to fat. Even when the mother has gestational diabetes, the fetus is able to produce all the insulin it needs. The combination of high levels of glucose in the blood of the mother and high insulin levels in the fetus results in large deposits of fat that causes the fetus to grow too large.
Birth Injury Birth injuries can occur due to the large size Baby and the difficulties of being born.
hypoglycemia Hypoglycemia refers to low blood sugar in the baby immediately after delivery. This problem occurs if the mother's levels of sugar in the blood have been consistently high, causing the fetus to a high insulin level circulation. After delivery, the baby continues to have a high insulin level, but no longer has the high level of sugar from its mother, resulting in newborn sugar in the blood are very low. The level of sugar in the baby's blood is checked after birth, and if the level is too low can be necessary to administer intravenously.
respiratory hazard (difficulty breathing) Too much insulin or too much glucose in the baby's system may delay lung maturation and cause respiratory difficulties in babies. This is most likely that if born before 37 weeks pregnancy.
High blood pressure and pregnancy:
High blood pressure can occur in pregnancy in two ways. It may be a pre-existing condition, called chronic hypertension, or may develop during pregnancy - a condition known as hypertension induced by pregnancy (PIH). It is also called toxemia or preeclampsia. PIH occurs most often in young women with a first pregnancy. It is more common in twin pregnancies and in women who had preeclampsia in a previous pregnancy.
and high bloodpressure can lead to placental complications and fetal growth retardation. If untreated, severe hypertension may cause dangerous seizures and even death in the mother and fetus.
Women with mild preeclampsia may require bed rest. Moderate or severe PIH usually requires hospitalization and medications.
Women with high blood pressure often need to continue taking their antihypertensive medication. Your doctor may change to a more antihypertensive medication safe during pregnancy.
renal function tests and ultrasounds are often performed more frequently in women pregnant with high blood pressure to monitor the health of the mother and fetal growth and development.
Infectious diseases and pregnancy:
Infections during pregnancy may pose a threat to the fetus. Even a simple urinary tract infection, which is common during pregnancy, should be treated immediately. An untreated infection can lead to premature labor and rupture of the membranes surrounding the fetus. Some infectious diseases include:
toxoplasmosis Toxoplasmosis is an infection caused by single-celled parasite Toxoplasma gondii. Although many people may have Toxoplasma infection, very few have symptoms because the immune system usually keeps the parasite that causes the disease. Babies who became infected before birth can be born with serious mental or physical problems. Toxoplasmosis often causes flu-like symptoms, swollen lymph glands or muscle aches and pains that last a few days to several weeks. Mothers can be tested to see if they have developed antibodies disease. Fetal testing may include ultrasound, and / or examination of amniotic fluid or umbilical cord blood. Treatment may include antibiotics. The Centers for Disease Control and Prevention (CDC) recommends the following measures to help prevent toxoplasmosis infection:
Wear gloves when gardening or doing anything outdoors that involves handling soil. Cats may pass the parasite in their feces, often use gardens and boxes trash. Wash hands thoroughly with soap and warm water after outdoor activities, especially before eating or preparing any food.
Ask a healthy and not pregnant gearbox cat. If this is not possible, wear gloves and clean the litter box daily (the parasite found in cat feces only can infect a few days after approval). Wash hands thoroughly with soap and warm water afterwards.
Have someone who is healthy and not pregnant handle raw meat for you. If this is not possible, wear latex gloves when touching raw meat and wash the cutting board, sink, knives and other utensils that might have touched raw meat. Wash hands thoroughly with soap and warm water afterwards.
Cook meat thoroughly, that is, until no longer pink in center or until juices run clear. Do not sample meat before it is cooked.
food poisoning A pregnant woman should avoid eating undercooked or raw foods because of the risk of food poisoning. Food poisoning can dehydrate a mother and deprive the fetus of nourishment. In addition, food poisoning can cause meningitis and pneumonia in a fetus, resulting in death. Tips for preventing food poisoning include:
Thoroughly cook raw food from animal origin such as beef, pork or poultry.
Wash raw vegetables before eating.
Keep raw meats separate from vegetables and food cooked and ready to eat.
Avoid raw (unpasteurized) milk or foods made with raw milk.
Wash hands, knives and cutting boards after handling food raw.
STDs
chlamydia - infections such as chlamydia may be associated with preterm birth and rupture of the membranes.
hepatitis - inflammation of the liver, resulting in cell damage liver and destruction. There are five main types of hepatitis have been identified. The most common type that occurs during pregnancy is hepatitis B (HBV). This type of hepatitis spreads mainly through contaminated blood and blood products, sexual contact and contaminated intravenous needles. Although HBV resolves in most people, 10 percent have chronic HBV. Hepatitis B can cause chronic hepatitis, cirrhosis, liver cancer, liver failure and death. Infected pregnant women can transmit the virus to the fetus during pregnancy and childbirth.
The later in pregnancy a mother contracts the virus, the greater the likelihood of infection in your baby. Signs and symptoms of HBV include jaundice (Yellowing of the skin, eyes and mucous membranes), fatigue, abdominal pain, loss of appetite, intermittent nausea and vomiting. A blood test for hepatitis B is part of routine prenatal testing. HBV positive mothers may receive a medicine called immunoglobulin hepatitis B. Babies born to HBV positive mothers should receive hepatitis B immunoglobulin and hepatitis B vaccine within 12 hours after birth. The American Academy of Pediatrics (AAP) recommends that all babies receive a total of four doses of hepatitis B vaccine to be fully protected against infection with hepatitis B.
human immunodeficiency virus (HIV) - A woman with virus human immunodeficiency virus (HIV) has a one in four chance of infecting her fetus. AIDS (acquired immunodeficiency syndrome) is caused by the human immunodeficiency virus (HIV), which kills or impairs cells of the immune system and progressively destroys the body's ability to fight infections and certain cancers. The term AIDS applies to more advanced stages of HIV infection. HIV is most commonly spread by sexual contact with an infected partner.
HIV can be spread through contact with infected blood, especially by sharing needles, syringes or drug use equipment with someone who is infected with the virus. According to the transmission of the National Institutes of Health (NIH), HIV from mother to child during pregnancy, childbirth and childbirth or breastfeeding has been responsible for nearly all AIDS cases reported among U.S. children.
Some people may develop flu-like illness within a month or two after exposure to HIV, although many people do not develop any symptoms when infected. Persistent or severe symptoms may not appear for 10 years or more after HIV first enters the body in adults, or within two years in children born with HIV infection.
The Institute of Medicine, the American College of Obstetricians and Gynecologists, the American Academy of Pediatrics and other health organizations recommend HIV testing of all pregnant women. prenatal care that includes HIV counseling, testing and treatment for infected mothers and their children saves lives and resources. Current recommendations for HIV-positive mothers to take a number of drugs during pregnancy and childbirth. Blood tests are also done to check the amount of virus. Newborn born to HIV positive mothers also receive medication. Studies have found that this can reduce the chances of transmitting HIV from mother to baby from 25 percent less than 2 percent. Cesarean delivery may be recommended for HIV positive women. This also helps to reduce transmission to the baby, especially when the mother receives medications. HIV can be transmitted through breast milk. Studies show that breastfeeding increases the risk of HIV transmission about 14 percent.
herpes - genital herpes can be transmitted to baby during birth, if a woman has an active infection at that time. Herpes is a sexually transmitted disease caused by herpes simplex virus (HSV). Herpes infections can cause blisters and ulcers in the mouth or face (herpes oral), or in the genital area (genital herpes). HSV is a lifelong infection. Symptoms of HSV may include painful blisters or open sores in the genital area, which may be preceded by a tingling or burning sensation in the legs, buttocks, or genital region. Cold sores usually disappear within a few weeks, but virus remains in the body and the lesions may recur from time to time.
It is important that women avoid contracting herpes during pregnancy because a first episode during pregnancy poses a greater risk of transmission to the newborn. Women can be treated with an antiviral drug acyclovir if the disease is severe. Genital herpes can cause potentially fatal infections in babies if the mother has active genital herpes (shedding the virus) at the time of delivery. Cesarean delivery is usually recommended for active genital herpes. Fortunately, the infection of a child is rare among women infected genital herpes.
Protection against genital herpes includes abstaining from sex when symptoms are present, and the use of latex condoms between outbreaks.

This is an example of a discussion on the use of dietary supplements referred to the drug Fen-Phen:
The use of dietary supplements containing ephedra alkaloids may pose a health risk to some people. Dietary supplements containing ephedrine alkaloids (sometimes called ma huang) are widely promoted and used in the United States as a way to lose weight and boost energy.
In light of the recently reported adverse events related to the use of these products, the Food and Drug Administration (FDA) has proposed limits on the dose and duration of use of these supplements. The FDA requested an independent review of adverse event reports related to the use of supplements containing ephedra alkaloids to assess the relationship causality and to estimate the level of risk the use of these supplements poses to consumers.
We reviewed 140 reports of adverse reactions related to the use of supplements dietary supplements containing ephedrine alkaloids that were submitted to the FDA between June 1, 1997, and March 31, 1999. A standardized classification system for evaluation of causality is applied to each adverse event.
Thirty-one percent of the cases were considered definitely or probably related to the use of supplements containing ephedrine alkaloids, and 31 percent is considered to be possibly related. Among the adverse events were considered definitely, probably or possibly related with the use of supplements containing ephedrine alkaloids:
47 percent involved cardiovascular symptoms 18 percent involve the central nervous system. Hypertension was the single most frequent adverse effect (17 reports), followed by palpitations, tachycardia, or both (13), stroke (10) and seizures (7) Ten events resulted in death, and 13 events produced permanent disability, representing 26 percent of likely final, and possible cases.
No doubt the above discussion that should be more aware of the effects of this drug and the fact that litigation is ongoing.
